State law (
Water Code 13752) prohibits distributing well completion reports to anyone but the
landowner, his or her designee, or a government agency without the owner's permission.
If you wish to request a copy of a well completion report for a well that you own, you need to complete the
Owner Well Completion Report Request (PDF, 17KB) and mail or fax it to the appropriate
district office shown on the top of the form.
If you are not the owner of a well, there are two separate forms available: one for governmental
agencies and their authorized agents conducting studies, and one for persons conducting conducting
environmental cleanup studies. Please click on one of the links below to access the appropriate
